SNAP Support Facilitator

The Health & Welfare Council of Long Island (HWCLI) is seeking a dedicated community outreach professional to join our Targeted SNAP (Supplemental Nutrition Assistance Program) team. This role is ideal for someone who enjoys working directly with people, believes in equity and access to food, and thrives in both community and partner-facing environments. This facilitator will spend much of their time in the field at schools, clinics, and community sites, helping clients apply for SNAP benefits and educating partners about eligibility and resources. The position also involves collaborating with agency partners and community based organizations to strengthen referral systems and inform advocacy efforts that advance food security across Long Island.

Job Description:

The Health and Welfare Council of Long Island (HWCLI) is a private, nonprofit health and human services planning, research, public education, and advocacy organization serving as the umbrella for public and voluntary agencies supporting Long Island’s vulnerable individuals and families. HWCLI’s mission is to respond to the needs of Long Island’s most vulnerable populations, ensuring that the voices of the powerless are heard.

Responsibilities:

  • Educate clients about SNAP benefits, conduct prescreening, and provide application assistance
  • Partner with other human service agencies to create referral systems that connect clients with additional services
  • Train partner agencies on basic SNAP eligibility to ensure effective referral systems
  • Maintain expertise in eligibility, enrollment, and program specifications
  • Complete all required reports accurately and on time
  • Plan, coordinate, and conduct presentations and workshops in the community
  • Develop targeted outreach materials tailored to specific demographics
  • Inform HWCLI’s advocacy and policy work through interactions with community members and trends in the field
  • Attend regularly scheduled sessions for program information, updates, ongoing training, and technical support

Qualifications, Requirements & Experience:

  • Excellent organizational skills and attention to detail
  • Strong interpersonal skills and comfort working with diverse communities
  • Excellent written and oral communication skills
  • Bilingual strongly preferred
  • Computer literacy required
  • Vehicle required for travel throughout Long Island
  • A/B.S preferred; significant relevant work experience will be considered
  • Able to work some nights/weekends as needed

Salary Range: $45,000 – 48,000

Job Type: Full time

Schedule:

  • 8 hour shift
  • Monday to Friday
  • Weekends as needed
