The Health and Welfare Council of Long Island (HWCLI) is a private, not for profit, health and human services planning, research/public education and advocacy organization that serves as the umbrella for public and non-profit agencies serving Long Island’s poor and vulnerable individuals and families.

The Health Equity Alliance of Long Island (HEALI) is Long Island’s Social Care Network (SCN) of community-based organizations (CBOs) and healthcare providers building a person centric model that integrates healthcare, social care, and behavioral health care. HWCLI is the lead entity for the HEALI Social Care Network. The SCN brings together social service and health care providers from across Nassau and Suffolk counties through identification, care coordination, integration, and provision of tailored funding to provide enhanced healthcare equity.

JOB ANNOUNCEMENT: Manager of Performance and Technical Assistance, Social Care Network

HWCLI seeks an innovative, detail-oriented, and socially conscious individual to support the HWCLI and HEALI missions by overseeing the delivery of performance enhancement and technical assistance activities for the Social Care Network (SCN). A successful candidate should be equipped with expertise in the areas of social services navigation and delivery, as well as quality and data. The Manager will ensure network organizations across Long Island receive quality improvement and change management support to enhance the network capacity and improve individual member outcomes.The SCN is part of New York Medicaid’s Section 1115 Waiver demonstration designed to identify individuals with health related social needs (HRSNs) and coordinate social care delivery in various regions across the state. This is a new Manger-level position, funded through an award from the state to support the implementation of the SCN. The Performance and TA Manager will report directly to Director of Performance Measurement

The core responsibility of the Manager of Performance and TA position will be to meaningfully assist and oversee a team of Coordinators and, together, provide in-person and hybrid training and technical support to SCN organizations across Long Island. The Manager will also work with the HEALI Network Development and Data Analytics teams to identify and respond to trends, needs, and disparities. This role requires a high level of adaptive leadership to steward a culture of data-driven care management and quality improvement across a network of partner organizations.

Responsibilities include: 

  • Oversee a team of Performance Measurement Coordinators who will each provide analysis, training, and technical assistance to the SCN member organizations;
  • Support the development and implementation of network-wide performance monitoring strategy;
  • Contribute to the design and develop of key programmatic milestones, policies, and procedures;
  • Develop and Implement data collection tools and processes for performance monitoring;
  • Design and provide tailored and client-centered technical assistance and training to network partners to ensure the adoption and compliance with policies and procedures consistent with the performance monitoring requirements of the SCN Waiver;
  • Develop and facilitate capacity building activities for CBOs and other network providers, with a primary focus on user onboarding and training, technical assistance, and systems integration;
  • Engage in continuous quality improvement activities by capturing and building upon lessons learned;
  • Support facilitation of the CBO Training and Capacity Building Subcommittee;
  • Engage in hiring, onboarding, and retention activities for HEALI;
  • Assist with developing reports to HEALI Governing Body and NYS DOH;
  • Represent SCN Leadership at meetings and other events, as requested;
  • Perform other duties and functions as assigned.

Qualifications and Experience

  • Bachelor’s degree in a related field. Master’s degree preferred;
  • 3-5 years experience in a project management, managerial, or supervisory role; preferably in the public health or social service sector;
  • Experience working with Unite Us platform or similar referral and care management tools preferred;
  • Prior experience in issues related to healthcare; social services; health equity, access and quality; population health (or some subset/combination of these) preferred;
  •  Demonstrated ability in successfully engaging in complex, large-scale performance improvement or change management projects in a fast-paced, dynamic environment;
  • Valid NYS driver’s license and the ability to travel locally in Long Island as needed for in-person meetings.

Desired Skills

  • Adept with data analysis and visualization;
  • Strong active listener and creative problem solver;
  • Strong knowledge of QI methodologies, capacity building strategy, and/or project management;
  • Strong interpersonal skills with the ability to tailor style to match audience;
  • Able to work collaboratively across an interdisciplinary team, managing multiple priorities;
  • Experience with healthcare quality measures and standards (e.g., HEDIS) preferred;
  • Experience with population health/referral management systems design and/or architectures strongly preferred;
  • Familiarity with state and federal information privacy regulations (e.g., HIPAA) and its impact on data systems and data management;
  • Strong commitment to social justice and HWCLI’s mission;
  • Proficient in Microsoft Office and Google Workspace.

Benefits:

  • Salary range: $85,000.00-$100,000.00/year
